Create an EOV Form Based on an Existing One

1.   If you haven't already, go to the Custom EOV Form Builder module.

2.   If necessary, use the filter buttons to locate the form you want to use for your new EOV form. See Using Custom Filters for details.

3.   Click the name of the form to display it in Edit mode.

4.   Update details for the form according to its type:

• Summary

  • To include additional EOV data, click the at the Section field and make your selection. For each section you selected, add fields that you want reported. Do so by clicking the section name to display a picklist of associated fields. Then, click checkboxes, as desired. You can use the Labels and Text options to add formatting (such as bold, italics, and underlining) between sections on your form.

  • To remove previously selected items, click . To reorder items, click the up or down arrows in the Move column.

  • To select another heading for this form, click the  at the Heading field and make your selection. See Working with Headers for EOV Forms for details.

• Patient Summary

  • To include additional EOV data, click the at the Section field and make your selection. For each section you selected, add fields that you want reported. Do so by clicking the section name to display a picklist of associated fields. Then, click checkboxes, as desired. You can use the Labels and Text options to add formatting (such as bold, italics, and underlining) between sections on your form.

  • To remove previously selected items, click . To reorder items, click the up or down arrows in the Move column.

  • To select another heading for this form, click the  at the Heading field and make your selection. See Working with Headers for EOV Forms for details.

• Billing

  • To include additional EOV data, click the at the Section field and make your selection. For each section you selected, add fields that you want reported. Do so by clicking the section name to display a picklist of associated fields. Then, click checkboxes, as desired. You can use the Labels and Text options to add formatting (such as bold, italics, and underlining) between sections on your form.

  • To remove previously selected items, click . To reorder items, click the up or down arrows in the Move column.

  • -   To select another heading for this form, click the  at the Heading field and make your selection. See Working with Headers for EOV Forms for details.

  • To update the institution associated with this billing form, click the at the Billing field and make your selection.

• Order Requisition

  • To include additional EOV data, click the at the Section field and make your selection. For each section you selected, add fields that you want reported. Do so by clicking the section name to display a picklist of associated fields. Then, click checkboxes, as desired. You can use the Labels and Text options to add formatting (such as bold, italics, and underlining) between sections on your form.

  • To remove previously selected items, click . To reorder items, click the up or down arrows in the Move column.

  • To select another heading for this form, click the  at the Heading field and make your selection. See Working with Headers for EOV Forms for details.

  • To update the location associated with this order requisition, click the at the Lab field and make your selection.

5.   Click Save.

6.   Type an EOV Form Name. Then, select whether you want to save your custom EOV form to your personal list ( My List) or make it available to others within your practice ( Practice or Practice General).

7.   If desired, click Save to Modules.

8.   Click Save As New.
