Advance Care Planning Overview

The Advance Care Planning Summary module consolidates advance care planning information in a single location. It provides a central access point for a patient's Code Status, EOL Referral, Advance Directive, and Values and Goals information.

Note: This module replaces the Code Status and EOL Referrals options in the Patient Chart menu. Additionally, Custom Summaries that included tabs for Code Status or EOL Referrals are replaced by the ACP Summary tab.

A table is displayed with a list of documents for each type of advance care planning information. If you or another LMR provider has entered information for a document, columns displayed include the date entered and user who made the entry. Clicking the link for the entry displays the form for viewing or updating.

For Code Status information only, a separate line displays for each site (institution) set up to document Code Status. This information is to be referred to in the event of an emergent situation in the outpatient setting. If applicable, click History form the detail screen to display the current DNR/DNI information with "Date Entered" links to view previous entries.

Click Values and Goals at the top of the screen to display a new table with information on the following categories of information: Prognostic Understanding, Information Preferences, Prognostic Communication, Goals, Fears/Worries, Unacceptable Function, Tradeoffs, and Family.
