Order Neonatal Physiologic Monitoring and Checks—Vitals

• The default options and settings are determined by age group. To override the age group for a neonate, see Changing a Neonate Age For Orders.

• If you haven't already:

1. Select the patient.

2. Click Write (ALT+W).

3. Click VS/Monitoring/Restraints/Code Status (ALT+V).

4. Click Physiologic Monitoring and Checks (ALT+M).

• Complete the form

• Click Neonatal Guidelines. for information on alarm limits, based on gestational age. Click OK to return to the order form.

• Click More Info (ALT+I) for clinical guidelines on ECG and pulse oximetry monitoring. Click the CAS icon in the taskbar at the bottom of the monitor to toggle back to Order Entry. Click  to close the window.

5. Click a checkbox (or press ALT+the underlined letter) to select one or more monitor or check options.

6. Complete the selections as follows:

• Cardiac Monitoring (ALT+E): Click to select a risk option, based on the descriptions provided.

• O2 Saturation Monitoring (ALT+U): Click to select a risk option, based on the descriptions provided. See below at O2 Sat Monitoring for setting SpO2 alarm limits.

• Routine Cardiac / Respiratory Monitor (ALT+R): Type numbers to set the upper and lower limits for both the heart rate (bpm = beats per minute) and respiratory rate (bpm = breaths per minute).

• O2 Sat Monitoring (ALT+O): Type numbers to set the upper and lower limits for the SpO2 alarm.

• Pain Assessment - NPASS tool (ALT+P): Click Select a frequency. If Other, type a frequency in the field provided.

• Neuro Checks (ALT+N): Select a frequency. If Other, press TAB and type a frequency in the field provided.

8. Select a Time (Now or Other ).

9. Click OK (ALT+O).

10. If the patient already has an ECG or Pulse Oximetry Monitoring order:

• Click Cancel New Order (ALT+N) to leave the existing order as is.

• Click Keep New Order (ALT+K) to discontinue the existing order and continue with the new order.

• Write additional orders as needed, then sign the session.
