Discharge Meds - PAML or Inpatient Meds Changed

Since the most recent save (either preliminary or final) of the patient's discharge orders, one or both sources of the medications listed for reconciliation has changed. Please check whether these changes make it necessary to update the discharge orders.

Either of the following changes will trigger this message:

• The patient's pre-admission medications list (PAML) was updated; this could include the removal or addition of a medication, or edits to the dosing, frequency, planned action on admission, etc.

• The patient's current inpatient medications were changed; this could include the addition of an order, discontinuing an order, or edits to an order
