Transfer orders are written in a distinct session that allows you to add new orders as well as view and edit existing ones.
The sending team or unit writes transfer orders for use after transfer. Whenever you write orders after a transfer session is signed, you are given the option to leave the new orders inactive (to be included in the transfer session), or to make them active for current use.
If the patient has been admitted from the ED without pre-admission orders, and there are still active orders from the ED, you will be prompted to inactivate the ED orders before continuing.
If the allergy record was updated in the ED, the order(s) must be resigned. MDs, NPs, PAs, and CNMs are authorized to resign. RNs and other staff, who are not authorized to resign, must notify an authorized provider that the resign is required.
If you time out or exit a transfer session without signing, the orders must be rewritten— they do not appear in Resume sessions.
Chemotherapy and chemotherapy-related orders are carried forward automatically when transfer orders are activated. Active orders for total/peripheral parenteral nutrition (TPN/PPN) are also continued automatically.
The receiving care unit must activate the orders when the patient arrives. If the patient never arrives and transfer orders have been activated, only the transfer orders are active. Any previous orders are inactivated.
The status code in the UCM for inactive transfer orders is "Transfer." Once the orders are activated, the status changes to "Pending."
Transfer orders are expired if not activated within 36 hours after they were signed. Expired transfer orders are discarded automatically when a new set is started for the admission.
Transfer orders can be discarded before activation, but once activated must be discontinued the same as any other active orders.
