Order Labels for the ED administration record—Nurses in the main ED treatment areas (but not ED observation) do not transcribe orders manually to paper. Orders in the main ED that must be documented on the ED administration record automatically generate sticky labels. The orders must be signed as reviewed (see RN Review and Signing Verbal Orders for procedures). After the order has been signed as reviewed by an RN, labels print for the following:
Medications
IV fluids
Blood transfusion
O2 and aerosol therapy
Life-sustaining Treatment (code status
Allergies (as needed)
Label sheets—Once all of the orders print, the printer cuts the label sheet. Each sheet starts with a header label that identifies the patient by name, medical record number (MRN), date of birth, age, and gender. One or more labels is generated for each order (see multi-label orders below). The end label at the bottom of the sheet Includes the patient's name and MRN. The end label is also marked "***End of Label Print***" and includes the number of labels printed for the session (orders only, excluding the header and end) and the name of the RN or NP who triggered the printout. Each label on the sheet is marked in the lower right corner with a two-letter key that is unique for orders from this print session for the selected patient. (It is matched to the MRN for the printout.) This label key provides a way to quickly scan the sheet to see that all of the orders do in fact belong to the same patient.
Allergies—There is a space for the allergy record at the top of the document, beside the patient's hospital card imprint. Allergy labels print automatically when an RN signs the first review session for the current ED visit, and again when an RN signs the review of updates to the allergy record made in ED during current visit. Paste an updated record over the older label.
Labeling the ED administration record—Peel off the label for each order on the sheet and attach each to the ED administration record. Discard the header and end labels. Remember to dispose of materials that identify patients properly to ensure confidentiality.
Multi-label orders—Order labels include the summary description, administration information, the POE order number, and the date and time ordered. If needed, more than one label will print to include all of the relevant information. These multiple labels are all marked in white on black "Continued [label number of total number of labels for this order]". For example, an order with three (3) labels would include "Continued [1 of 3]" on the first label, "Continued [2 of 3]" on the second, and "Continued [3 of 3]" on the on the last. Attach all of the labels, next to each other and in order, to the ED administration record.
Reprint labels—RNs and NPs can reprint order labels as needed. The header and end labels are marked "Reprint Session" in white on black. See Reprinting labels in the ED for the procedure.
