All patients admitted to MGH must have a pressure ulcer assessment documented in POE by an MD, NP, PA, or CNM. See the Overview of Pressure Ulcer Screening.
Providers writing orders are prompted at 6, 12, and 18 hours after the patient is admitted, until the documentation is complete.
The option to document the pressure ulcer assessment is selected by default. To complete the order:
Click OK (ALT+O). The Screening form opens automatically.
Select an option from the drop-down list to answer the question of whether a pressure ulcer is present on admission.
If Yes, remember to add a description of the wound to the admission note.
If Unknown, providers writing additional orders for this patient will be periodically prompted to update the order.
Click OK (ALT+O).
If you answer Yes, you are prompted to order a consult for pressure ulcer treatment. See Consequent Orders Overview.
If the consult is needed, click the checkbox to the left to select the order, then click Yes (ALT+Y).
If the consult is NOT needed, click No (ALT+N).
To defer:
Click to select the option to defer the assessment.
If the assessment is not documented by 18 hours after admission, the option to defer is no longer allowed.
Providers writing additional orders for this patient will be periodically prompted to complete the order.
Click OK (ALT+O).
