When writing a new order, you will be prompted to complete the referral information automatically.
To edit an existing order, click the button to the right of the service field.
For medical references from Anticoagulation Management Services (AMS), click AMS Documents (ALT+D).
Complete the referral information:
Click to select the primary indication for anticoagulation treatment from the drop-down list.
If applicable, click to select one or more secondary indications from the checkbox list. (Click again to deselect an item.) Scroll to view the entire list. For Other, type a description in the field beneath the list.
To include additional information as applicable, type in the field for relevant clinical info.
Select an option from the Service requested drop-down list.
Click the select an option for the patient's INR Goal and Therapy Duration.
Select an option for long-term management:
MGH physician: Look up the physician by name.
Non-MGH physician/specialist: Type the name and phone number of the provider. Type T for today, or select a future date from the calendar.
Click Yes, No, or Unknown to indicate whether the patient requires an interpreter, and whether the patient will be followed by visiting nurses (VNA).
Click OK (ALT+O).
Complete the Consult form:
Select a time (Now, Today, Tomorrow, or Other).
The anticipated date of discharge is carried over from the referral page.
Click OK (ALT+O).
Write additional orders as needed, then sign the session.
