May 13, 2015—Enhancements to the Medication Source List in the PAML Builder
The pre-admission medication list builder (PAML) has been enhanced to include additional electronic data sources where available. Along with previously available data from the active medications list in the LMR, the Oncall medical record, and recent MGH and BWH discharge orders, records from the BWH eCare discharge orders and continuity of care documentation (CCDA) from outside institutions are now included.
In addition, with the consent of the patient or caregiver, data from Surescripts© is included when available. Surescripts© includes both insurance claims and pharmacy dispense records.
The medication source list (MSL) has also been enhanced to aggregate the available data. Multiple records for a treatment—a discharge order, the same medication on the patient's LMR active meds list, and a Surescripts© prescription—are all grouped together, with the most recent and/complete record as the heading. Only one record from the same source is included.
Key features:
The patient or caregiver must consent to allow access to Surescripts© data. If you do not have consent on starting the PAML, the MSL opens without any Surescripts© records. If you obtain consent later, you can document this, and the Surescripts© information will be added to the MSL.
Any clinician with authorization to build the PAML can document the patient's consent.

in a collapsed list. Clicking the
to the left of the heading displays the related records.
Instructions:
On starting the pre-admission medications list (PAML), you are prompted to indicate whether the patient and/or caregiver has consented to allow access to Surescripts© data on prescriptions.
Click OK if you obtained consent. With consent, any available Surescripts© data is included in the medication source list.
If you have not been able to obtain consent, click Cancel. The PAML builder will open, but the medication source list will not include Surescripts© data.
If you obtain consent after the PAML has been started, open the PAML, then click the patient consent link in the note below the Meds from Electronic Sources title to reopen the consent prompt.
