August 4, 2015—Discharge order information for pharmacy
Pharmacy staff can access a new folder in discharge orders to review orders for medications at discharge and the pre-admission medications list (PAML), along with current inpatient medications that are not ordered for discharge.
Key features:
The list of discharge medications indicates whether and where a prescription was printed, for each medication.
Pharmacists can enter comments to include general pharmacy information on the Post-Hospital Patient Care Plan (PHPCP).
Pharmacists can also enter instructions for the patient, similar to the separate provider and nursing instructions, for any discharge medication order. If there are pharmacy instructions, the order is flagged with <PI> in the Inst column. These print on the Face Sheet, Patient Care Referral, Nursing Discharge Note, and Patient Discharge Meds List (PDML).
