August 4, 2015—Discharge Core Measure Monitoring For Heart Failure Target Dry Weight
To comply with the guidelines for care of heart failure patients and earn recognition from the American Heart Association, additional information is required for core measure monitoring at discharge.
Key features:
This monitoring affects patients with all heart failure diagnoses except atrial fibrillation, and patients with a diagnosis of atrial fibrillation when the discharging provider indicates the patient does have a history or symptoms of heart failure on the Core Measures form.
When the patient has a history or symptoms of heart failure, the ordering provider is required to enter the target dry weight (lbs) for the patient. This information is included in the PHPCP.
