September 29, 2014—Core Measure Documentation at Discharge for Pneumonia
To comply with the redesigned guidelines for care of patients with pneumonia, discharge orders have been updated to include core monitoring documentation for clinicians.
Key features:
This monitoring affects all adult patients whose principal or associated diagnoses on the Face Sheet is any one of over 30 pneumonia types.
For all adult patients with a principal diagnosis of pneumonia, the core measure "Does the patient have a diagnosis of pneumonia during this admission?" is automatically answered Yes; this cannot be changed.
For all adult patients with an associated diagnosis of pneumonia, the core measure "Does the patient have a diagnosis of pneumonia during this admission?" must be answered by the provider writing discharge orders.
When the core measure is answered Yes, the discharging nurse is required to document on the Post Hospital Patient Care Plan (PHPCP) whether the patient has received patient education materials for home care of pneumonia.
