March 18, 2014—Discharge Alerts When Inpatient or PAML Medications Change

Once discharge orders have been started and saved (whether preliminary or final), clinicians who next access the orders are alerted if changes were made to either the pre-admission medications list (PAML) or the current inpatient medication orders. The alert reminds providers that the reconciliation of medications for discharge needs to be revisited.

Changes to the PAML that trigger the alert include the removal or addition of a medication, or edits to the dosing, frequency, planned action on admission, etc. For inpatient medications, the alert is triggered by  the addition of an order, discontinuing an order, or edits to an order.
