June 17, 2014—Enhancements to Discharge Core Measure Monitoring For Heart Failure
To comply with the guidelines for care of heart failure patients and earn recognition from the American Heart Association, additional information is required for core measure monitoring at discharge.
Key features:
This monitoring affects patients with all heart failure diagnoses except atrial fibrillation, and patients with a diagnosis of atrial fibrillation when the discharging provider indicates the patient does have a history or symptoms of heart failure on the Core Measures form.
For patients with a history or symptoms of heart failure on the Core Measures form, the discharging provider must answer the question "Does the patient have a LV Ejection Fraction of less than 40% or evidence of moderate to severe systolic dysfunction?"
If the answer to the question on the LV ejection fraction is Yes, the discharging provider must answer two (2) additional questions on medications: “Will patient be discharged on a beta-blocker considered to be evidence-based for heart failure therapy (Bisoprolol, Carvedilol, or Metoprolol Succinate (Extended Release))?” and “Will ACEI/ARBs be ordered for patient at discharge?”
If the answer to the question on evidence-based beta blockers is Yes, and none of the meds listed are ordered for discharge, the provider is alerted that an order is needed. The prompt provides direct access to the discharge medication orders list.
If the answer to either medication question is No, a reason is required.
