May 29, 2012—DEx Discharge Summary Notes for Liver and Renal Transplant Patients
For patients with a discharge diagnosis of transplant of liver or transplant of kidney, their discharge summaries in DEx (discharge express) is automatically set to the applicable type of note. (Providers can manually select a different note type if needed.) These notes, in addition to general information needed for all admissions, include sections for transplant-related concerns.
Key features:
Note type is set automatically based on patient's diagnosis for discharge (see Completing the Face Sheet, Primary Diagnosis)
Additional information includes general transplant data such as the type of graft, wound care, vascular concerns, episodes of rejection, immunosuppressive medications, infectious disease and prophylaxis, high risk donor, the mandatory multidisciplinary plan, clinical follow-up date, and care team contact information
For liver transplants, other information includes biliary concerns, renal function, and studies and tests of lung function
For renal transplants, other information includes stent, and studies and tests of kidney function
