December 11, 2012—Additional Information on the Face Sheet for Discharge
To meet 2013 requirements from the Massachusetts Department of Public Health, documentation for discharge must include additional information for the patient.
Key features on the Face Sheet:
The Face Sheet now includes an entry for the provider to contact with questions regarding the inpatient visit
The attending from the EMPI (Enterprise Master Patient Index) is the default provider to contact
A phone number from the MGH provider database is included if available
The default provider information can be corrected if needed
The ordering provider is reminded to list all significant operations, procedures, and tests performed during the inpatient visit, and to include dates an times for reference
Information Added to the Discharge Papers Given to the Patient:
Admitting primary diagnosis
Discharge principal diagnosis
Significant operations, procedures, and tests
Life-sustaining treatment (code status) at discharge
Provider to contact regarding hospital stay, with phone number when available
Pending Results at Discharge: If a pending result has been selected by a provider, it prints on this report. Otherwise, the section notes that the patient had no pending lab tests at the time of discharge
