Oral Chemo Medications at Discharge

Physicians, Nurse Practitioners, and Physician Assistants can order oral chemotherapy medications at Discharge by resuming the medication from the PAML (Pre-Admission Medication List). This applies only to 11 oral chemotherapy medications. The ordering clinician can edit the medication but cannot write a prescription. Clinicians cannot add a new chemo medication at discharge.
