May 2010
Starting in late May, several functions related to orders for restraint have been enhanced to ensure that a timely order is present for all patients who are in restraint. Restraint orders require a current assessment of the patient and of the behaviors that indicate the need for restraint. Restraint orders for potential conditions (PRN) are not allowed by MGH policy. Options are now arranged to make it easier for the responding clinician to write an order for either a single restraint, or a clinically appropriate combination of restraints.
Key features:
Restraint types have been divided into categories: bed, torso, mechanical limb, chair
Restraints can be ordered from more than one category as appropriate
A maximum of one restraint from each category can be ordered
By default, all orders include the instruction that the RN can discontinue the restraint if the patient is no longer exhibiting the behavior. The RN should notify the responding clinician when restraint is removed, and request the order to discontinue. This instruction can be removed if appropriate.
Restraint reminders include a prompt to reassess the need before reordering restraints
