Discharge Orders—Possible Unmatched Medication

 

You have selected a free-text  inpatient medication to continue on discharge. When a medication for inpatient treatment is entered as free-text, you should check the PAML to determine if the order also exists there. PAML records can be drawn from outpatient formularies, which can include medications that are not available from the inpatient formulary. When that is the case, to continue the medication in-house the formulary outpatient order must be written as a free-text medication.

The free-text inpatient order cannot be matched automatically to a PAML record. This can result in conflicting instructions on the patient's discharge medication list (PDML), which would say "take" the free-text inpatient medication and "stop taking" the identical PAML medication.

Check the PAML list in the discharge comparison. If there is a match for the free-text inpatient order, clicking Yes to continue from the PAML side and No not to continue the inpatient order will resolve correctly in the patient's discharge med list, because inpatient orders that are not continued are not included in the PDML.
