Completing the questionnaire - options for Chrome users

• If you don't already have it, add the Adobe Acrobat reader extension from the Chrome store:

4. Click this Archiving Considerations link to open the document in Chrome.

• If you don't want to add the Adobe Reader extension, open the form in Edge. Copy this URL https://clinicalhelp.partners.org/mgbArchiving/MGB_ArchivingConsiderations_v5.pdf into the address field and then press ENTER.

• If you can't use IE or Edge, you need to download the document and open it in Adobe Reader:

1. Click this Archiving Considerations link to open the document in Chrome.
2. Click to download a copy, and save it to your device.
3. Open the document in Adobe Acrobat Reader, which is an application on your Windows applications menu. (If you don't have the reader, download free here.)
4. Complete each section with as much information as possible.
5. Click to save the completed file for your records.
6. Click the Submit button at the end of the form.
7. If prompted, click to select the email application to use, and then click the Continue button.
8. The email opens, with the completed form attached. Send the message as addressed.
