Manage service provider information - general

4Next Provider Management allows authorized users to update the information that helps acute care providers, patients, and families decide which service provider is best suited for their post-acute care. The CMS Rating is displayed in the heading for every section. It is automatically refreshed from CMS data every week.

  • If you haven't already:
  1. Click Providers menu drop arrow on the main menu, and then click the Manage option.
  2. Click to select the site from the results.
  1. Click General in the index on the left.

Updating General information

  1. To update the top section, edit the fields as needed:
  • Name (required) - Type the provider's full name in the applicable text field.
  • Code - If applicable, type the code for the provider.
  • CMS rating - the CMS rating is refreshed automatically from data from the Centers for Medicare and Medicaid on a weekly basis. If needed, update manually by clicking to select a single option from the CMS Rating drop-down list.
  • To activate the provider in 4Next, click the Is Active switch to turn it on: toggle on
  • To inactivate the provider in 4Next, click the Is Active switch to turn it off: toggle off
  • License ID and License Name - Type the ID and name as recorded in the license in the applicable text field.
  • Parent Owned - If the provider is owned by a hospital, health care system, or other corporate entity, type the name of that organization in the Parent Owned text field.
  • Data Source - Type the name of the licensing entity in the text field.
  • Provider Type - Click to select an option from the drop-down listHospice At Home, Inpatient Hospice, Long Term Acute Care, Medical Equipment, Rehabilitation Hospital, Skilled Nursing Facility.
  • Referral method type - Click to select an option from the drop-down list: Fax, Online, or Unverified.
  • Management Company - If the provider is managed by a hospital, health care system or other corporate entity that is stored in the 4Next database, click to select an option from the drop-down list.
  1. Click save.

Adding provider classifications

  1. Click plus icon to display a checklist of options.
  2. Click to check checkbox checked one or more items. The only options that are available are those that are not already selected.
  3. Click save.

Removing provider classifications

Click the x on the classification to be removed.

Re-setting general provider information

To cancel the changes you made, click reset
