Working with the Provider Card

From the provider card, you can add the provider to your favorites list and print patient and care coordinator information sheets. Information on the provider card includes the full formal name (which is linked to detailed provider information), classification, CMS rating, favorites flag, provider type, referral method, phone and fax numbers (if available), website address link (if available), street address linked to google maps, and 'also known as' names.

  • Provider cards that are attached to a case are collapsed by default. Click the white space on the card Expand button to show details, click the top again Collapse button to hide details. 

Adding a provider to your favorites

  • To add the provider to your  Favorites list, click on the upper right of the provider card. Click to remove the provider from your  Favorites.

Printing provider information sheets

  • If the provider card is collapsed Expand button, click the white space on the card.
  • For a patient copy (doesn't include rates charged) click print button patient.
  • For a care coordinator's copy (includes rates charged) click print button care coordinator.
  • The document opens in a popup window. From there, you can print or save a copy.

  • Every page includes a footer with "Extracted from the Legal Health Record and printed on " and the date and time stamp, plus "Mass General Brigham Incorporated Clinical data printed from 4NEXT - Confidential" and the page number and page count.

  • Click × to close the popup.

Additional provider information 

  • To view provider information, including the date last updated:
  1. Click the provider name.
  2. Click the tabs at the top to view the different types of information.
    The CMS ratings update automatically every Monday at 1 a.m.
  3. To return to the provider card, click close.
  • To open the provider's online site in a new browser tab, click the website icon website address link. Not available for all providers.
  • Click the map icon street address link to to display the location in Google maps in a new browser window.
  • To return to 4Next:
  • In standalone, click X close tab to close the browser tab, or click the 4Next icon 4Next tab.
  • In Epic, click X close browser window to close the new browser window, and then click Epic taskbar icon in your taskbar.
  • Click the AKA icon AKA link to display a list of common name(s) for a facility which may have been previously known to the communities served. Click anywhere else to close the popup.
